RELEASE AND DISCLAIMER

I request procurement technical assistance counseling services from the Maine Procurement Technical Assistance Center (Maine PTAC) a federally and state funded program hosted at Eastern Maine Development Corporation (EMDC).  Maine PTAC services are provided at no charge to participating Maine businesses.  In exchange for these services:

ü    I agree to cooperate with Maine PTAC by responding to any surveys designed to evaluate Maine PTAC services that I am selected to participate in, regardless of whether I have obtained government contracts during the reporting period.
 

ü    I acknowledge that the information that I provide to Maine PTAC is current, accurate and complete to the best of my knowledge and belief as of the date of submission.
 

ü    I understand that it is my responsibility to market my business and that no contracts or sales

       can be guaranteed from Maine PTAC services and that these services do not constitute or take

       the place of legal or other professional advice or counsel.
 

In consideration of Maine PTAC’s furnishing government procurement or other technical assistance services, I waive any and all claims against the Maine PTAC, EMDC, their host organizations and their employees.

 

I understand that information provided to Maine PTAC will be held in strict confidence and only provided to federal and state agencies as required by law and to Congressional offices in order to promote the Maine PTAC program.  I further understand that all Maine PTAC counselors have agreed not to accept fees or commissions developing from this counseling relationship or to recommend goods or services from sources in which they have an interest.
